examen, enquête (RL)

contrôle, recherche, investigation- heavy and expensive process - only for the most serious events (accidents, serious incidents)- identification of the deep causes, - in order to eliminate these causes - in order to prevent from the event to reoccur
3 phases: 1) systemic collection of data on the event 2) analysis -> recommendations 3) corrective actions.
