Zollinger-Ellison Syndrome

Zollinger-Ellison syndrome is a disorder where increased levels of the hormone gastrin are produced, causing the stomach to produce excess hydrochloric acid. Often, the cause is a tumor (gastrinoma) of the duodenum or pancreas producing the hormone gastrin. Gastrin then causes an excessive production of acid which can lead to peptic ulcers (in almost 95% of patients)

A disorder in which tumors of the pancreatic islet cells produce large amounts of gastrin (a hormone), leading to excess acid in the stomach and, possibly, a peptic ulcer (ulcer of the stomach or the upper part of the small intestine).

(Gastrinoma)
These tomours arise mainly from G cells in the panceas and secret large amounts of gastrin.This stimulates maximal gastric acid secretion,so that main clinical problem is peptic ulceration. Peptic ulcers occur in usual area of the stomach and duodenum and in the jejunum. Haemorrhage and perforation can occur.
Diarrhoea resulting from the low pH in uper instestine is also common.A high serum gastrin confirms the diagnosis.
Treatment is with proton pump inhibitor e.g.Omeprazole.
Surgery is reserved for removal of the primary tomour only.These tomours are malignant and although they grow slowly the patients die of malignancy rathet than gastrointestinal problems if the primary tomour cannot be removed.
