( IBS )
This is a functional (psychosomatic) bowl disease.Examination of the patient reveals no organic abnormality.Motality abnormalities have been found in the irritable bowl syndrome,but these abnormal findings have not been consistent and do not always correlate with episodes of pain. Rectal examination and sigmoidoscopy should be performed.
Clinical features:
There is pain/abdominal discomfort classically situated in the left iliac fossa and is usually releived by defecation or passage of wind.The patient may complain of constipation or diarrhoea with the passage of frequent small volume stools and a feeling of incomplete emptying of rectum. Stool may be ribbon-like or rabbity in appearance.True watery diarrhoea suggest organic disease.The pain ,however,can be very variable and occur in any part of the abdomin and the bowel habbit may be normal.
The lenth of history is usually long with frequent recurrent episodes and long symptom-free intervals.
Management:
*Patient must be reassured of the benign nature of the condition.Cancer phobia must be dispelled
*A high-fibre diet or even a change in the diet help some patients.
*Small group of patients are depressed and improve with antidepressant therapy.
